criticize it for what it refuses to do and from taking the control of a patients health care out of the hands of physicians and placing it in the hands of bureaucrats who are interested only in revenue. Even selection of physicians and other health care services is dependent upon the HMO the patient subscribes to. They all have member-doctors and ancillary services and while some do allow a patient to choose someone outside of the network, that visit will cost more than accessing services that are in the network. Among the many different issues a patient experiences under managed care is the specific medications/drugs covered and the number of days a patient is allowed to stay in a hospital. These are not empty complaints, they are real and they are a detriment to the patients health. These rules can in no way be perceived as ethical since they put revenue as the first priority and the patients health as a second or third priority. It should be the opposite. The ethical approach is for physician or other health care provider to specify what is best for the patient and for the patient to be completely involved in that decision. As this author commented, there has been a great deal of literature about biomedical ethics relative to patient autonomy. This type of autonomy is limited, at best, with managed health care. This issue goes even further insofar as medical autonomy, in terms of professional health care providers, is limited. This takes the issue in the very broad area of social values, which is, in turn, about the values that need to guide a patients decisions as well as the providers decisions.
