EBSCO Host Research Databases, which is made available through the NC LIVE, a resource made available by N.C. State University and the Wake County Public Library, located in Raleigh, North
Carolina. Several databases, such Academic Search Premier and CINAHL with Full Text, were searched simultaneously using keywords, such as "disease management," "components," and "cost." Sources were then selected based on
their applicability to the topic. The analysis plan is to garner information pertaining to the topic, offer summation and then recommendations based on this information. Discussion of disease
management (DM) concept Disease management (DM) is defined as a "systematic clinical improvement process," which addresses both "provider and patient education employing national and locally developed best practice guidelines
and behavioral interventions" (Cohen, et al, 2001, p. 631). DM couples educational intervention with "close monitoring of clinical processes and outcomes of care" (Cohen, et al, 2001, p. 631). DM
