he or she has for treatment, which may or may not be realized by the health care provider. The second is the viewpoint with regard to the system as a
whole, and whether or not it meets peoples expectations. With regard to matching the expectations of people to the system as a whole, McGinley notes that while the poor aspire
to decent health care, the reality is that "health care for the poor is neither equal nor quality" (1994). For those who expect government assistance, she points out that "Medicaid
is not extended to all poor people" (McGinley, 1994). Even more unsettling is the fact that "public hospital emergency rooms are only required to perform a standard screening, not full
treatment of a patient" (McGinley, 1994). Many of these hospitals are leaving the inner cities, which leaves "many of the sick poor untreated and untreatable" (McGinley, 1994). A study from
Australia talks about aspirations in health care for one particular group, those with spina bifida who have survived into adulthood. Members of this group are already marginalized by virtue of
having the condition; their aspirations therefore are lower than for others, because "people who are classified as being within devalued and stereotyped groups tend to internalize and take on devalued
beliefs about, and expectations for, themselves" (Thesis, 2000, p. 69). People like this also respond to the discrepancies between aspirations and reality in a specific way: they tend to "adjust
their aspirations to their circumstances in order to reduce discomfort, because over time they have learnt that it is pointless to try to change the external circumstances" (Thesis, 2000, p.
