In the relatively recent past, just a century ago in the early 1900s, dying and death were integral factors in human experience--at any age--as death could be swift, usually occurring
after a short illness and could happen at any age (Bern-Klug, Gessert and Forbes 38). Today, this situation has changed as death primarily occurs in old age and within an
institutional environment. Similarly, it is no longer usual for an individual to live to old age. Modern medicine has also prolonged the period of decline in the elderly so
the younger generation is confronted with the problem of dealing with an elderly parent for possibly decades, in which these seniors require extraordinary care. It falls upon social workers and
others connected with family counseling to be the key "context interpreters" for aiding people in negotiating the issues that occur at the end of life. Additionally, these professionals are instrumental
in locating needed resources and in helping families develop needed in skills in handling the problems associated with old age. Due to advancements in medicine over the last century,
people in industrialized countries, rather than facing an early death from acute illness, usually die from a decades long process that involves chronic illness (Bern-Klug, Gessert and Forbes 38). This
process can have tremendous detrimental effects on families, both from a financial standpoint and an emotional one. Results from the SUPPPORT study (The Study to Understand Prognoses and Preferences for
Outcomes and Risks of Treatment) showed that far too many people died with unrelieved pain and symptoms, which was due to healthcare professionals failing to understand the patients desires concerning
