pressure was normal, but his pulse was erratic and he had no fever. Biographical and Demographical data: J.D. is of medium height, 510" and weight 180 lbs., which is
a little too much weight for his frame, but he is not obese. His stated age fits with his appearance. His speech was clear and his affect was normal, but
a little tense. J.D. lives with his wife of 32 years in Texas City, Texas and there relationship appears to be good. They have two adult children who are no
longer living in the home; both sons, Adam, 26 who live with his wife and children in Houston, and Michael, 22 who is single and in college. He is a
retired accountant who was never in the military and has never traveled out of country. Current Health status: J.D.s health has been very good all his life. As a
child, he remembers having mumps, chicken pox and measles, and was vaccinated for polio and DPT. He does not recall his last immunization booster for tetanus. He has never been
hospitalized nor had any major broken bones, but his right arm was once dislocated while playing football. To the best of his knowledge, he has no food, drug or airborne
allergies. He has never been involved in a serious industrial or automobile accident (Physical assessment, 2007). He is not taking any medications on a regular basis, just over-the-counter analgesics for
occasional headaches and aches-and-pains that J.D. associates with aging. J.D. reports that he uses tobacco products and alcohol products. He smokes roughly a pack of cigarettes per day and
