re-entry into the Earths atmosphere killing all seven crew members on board. The Columbia Space Shuttle disaster occurred because during the launch, a piece of foam insulation broke off and
damaged the shuttles thermal protection system (TPS). The TPS protects the shuttle from excessive heat that it ultimately experiences during re-entry. NASA personnel were aware of the large debris strike
and the Board made several requests to secure images of the space shuttle during its 16-day mission. Despite the tragedy that shocked the nation of the Columbia Space Shuttle disaster,
former President George Bush assured the American public that space exploration, albeit the dangers involved, would continue on. Nonetheless, in the wake of the crash, observers asked whether human space
flight was worth the risk. In addition, NASA understood that it would have to reassess the Columbia Space Shuttle disaster and find out exactly what went wrong as well as
look at changes and face the resistance that comes with those changes. For example, NASA engineers had made recommendations to the board immediately after the Columbia Space Shuttle disaster and
their investigation of the breakup of the space shuttle. In their report, they included changes to avert hazards like the fatal damage to Columbias wing, along with methods for finding
and even repairing damage in orbit. They also recommended that during future liftoffs that NASA watch closely for falling foam or other debris that could damage the orbiter and potentially
