Those who do not change will soon be out of business whether it is a public or private organization. It is also true regardless of industry. As Thompson (2010) points out, however, is that health organizations are especially under heavy scrutiny today. Consumers are demanding greater quality, lower costs, and higher satisfaction. Change is difficult in any organization. It may be more difficult in public organizations because, since it is a government agency, there are more pressures coming from more places. They are faced with political, legal and social forces while private health care organizations face legal and social forces but not the political pressures. Private companies do not face funding cutbacks from legislators as do public health organizations. Thompson offers a comprehensive overview of these and other aspects of managing change in public health organizations beginning with the fact that it is outside or external forces that shape public health organizations. It is important for public health managers to be able to forecast the dynamics outside the organization. Recently, these organizations have had repeated budget cuts and increased demands for accountability. Forward looking managers should have been able to forecast these factors. As consumers became more demanding, it was only reasonable that greater accountability would have been an issue for these organizations. Both these factors would lead to unplanned and urgent change.
