other physical and psychiatric disorders, including mood disorders (both depression and bipolar disorder), brain diseases, drug intoxication, and metabolic disorders (Binda (1995), 8). Like other forms of schizophrenia, catatonic schizophrenia is difficult to diagnose. The same symptoms of catatonia may be due to causes other than schizophrenia, and under the guidelines of the Diagnostic and Statistical Manual of Mental Disorders, now in its fourth edition (DSM-IV), gives specific criteria which must all be met in order to render a diagnosis of catatonic schizophrenia. Nevertheless, the diagnosis is still left up to the subjective judgment of the particular practitioner. There are no tests, medical or otherwise, which will lead to certain diagnosis. And indeed, there is no single symptom or set of symptoms that definitively defines its presence. Any two schizophrenic catatonics may have different symptoms, course of illness, and response to treatment (Keefe (1994), 21). It is therefore crucial first of all that the symptoms be accurately defined so that there is a standard against which to judge, and secondly that they be accurately described (Torrey (1995), 41). And to complicate the matter, there are other diagnostic systems besides the DSM-IV. The ICD-10 system is being touted internationally by the World Health Organization as the acceptable criteria for determining the presence of catatonic schizophrenia. The two systems are similar, varying primarily in how long they require symptoms to be present. Both require that he general criteria for schizophrenia be met as well as the special criteria for catatonia.
