not feel well and that she was diagnosed several years ago with type 2 diabetes mellitus, but never adhered to the recommendations for care. Her principal complaints are that she
has weakness in her right foot and a persistent rash in her groin area. Discomfort in her feet is particularly problematic in the evening. She is experiencing increased thirst and
has urinate more frequently during the night. She reports that the rash has been periodic problem for many years, worsening in warm weather and sometimes affecting her armpits. The patient
reports a family history of diabetes. Although she was told that her blood glucose level was around 200, she never followed care recommendations because she was asymptomatic till the foot
problem developed. She has gained 10 pounds over the last year, eats a high-fat, high-sugar diet and has a sedentary lifestyle. The objective data is what the attending nurse
observed, as well as the laboratory results on this patient. Basically, the patients physical parameters are all within normal ranges, with some exceptions. At her first physical examination, her blood
pressure was 165/96, which indicates that intervention is required. While her BP was lower on her return visit (150/102), this reading still indicates that this should be addressed. The patients
cholesterol and triglyceride level was also above normal to an extent indicating the necessity for intervention. The most disturbing laboratory result was the patients glucose level, which was 253 mg/dL
for a random reading, 170 mg/dL fasting glucose and a HgA1c of 9.1 percent, as this confirmed that she had uncontrolled type 2 diabetes. The nurse also noted that
