properly! 1. Project Description Clear & Concise Cain and Mueller are reporting on a project that was designed to improve the quality
of care in nursing homes. Traditionally, efforts that addressed the quality of care provided to the elderly in nursing homes was based on quality assurance (QA) models. This model focused
on the structure of the facility and processes used. It does not focus on outcomes. In recent years, at least some nursing homes have moved from a quality assurance approach
to a quality improvement (QI) approach. This is a more proactive model in that the entire focus is on improving the quality of care through planned actions. A demonstration
project was initiated in Minnesota by the Department of Human Services (DHS). Also, those involved changed to an alternative payment system (APS), which changes DHSs role from a payer to
a buyer of services for Medicaid residents. The new system directly links quality to payment. The APS provides both more flexibility to the nursing homes as well as greater accountability.
Accountability is achieved through the requirement for the nursing home to submit two quality improvement plans - one addressing a quality of care issue and the other addressing a quality
of life issue. In 1995, 40 nursing homes in Minnesota were selected to participate in the APS pilot project. By 2001, more than 70 percent of the facilities are involved.
This is an ongoing project in the state. All staff members plus family members and the resident, when possible, are involved in the planning and implementation process of (QI)
