Essay / Research Paper Abstract
A 3 page article critique that summarizes and analyzes a qualitative research study (Cook, et al, 2002), which investigated why ICU personnel do not use the semirecumbent position while administering mechanical ventilation as this has been shown to reduce the incident of pneumonia. Overview of the Article Empirical research has shown that placing patients in a semirecumbent position, compared with a supine position, is significantly associated with less gastroesophageal aspiration and pneumonia
in patients receiving mechanical ventilation. However, despite this fact, semirecumbency is not consistently used in practice. Therefore, the goal of this qualitative study was to investigate the perspectives of intensive
care staff regarding their views on the determinants and consequence of semirecumbency. Interventions or Key variables This heading does not apply to this study, as it was qualitative in
design. Important or Statistically Significant Findings As is typical of qualitative research, this study did not yield statistically significant findings that can be extrapolated to other settings. However, the
researchers did determine guidelines for semirecumbency use that are significant. For example, if a semirecumbency policy is adopted by the ICUs in the study, the researchers stress that perceived barriers
to its use must be understood and addressed. For instance, a true contraindication, such as cervical spine instability, must be decisively indicated as an exclusion criterion. Brief Overview of
other outcomes While this study does not specifically address semirecumbency as a focus for study, the assumption that it is a beneficial strategy in decreasing the incidence of ventilator associated
pneumonia is inherent to the research design. Therefore, the researchers begin their article with a summation of empirical evidence that verifies this position. Reliability concerns The researchers state that
data was analyzed inductively and in triplicate throughout the study, using a framework provided by identified emergent themes to provide abstract perceptions and meaning Recognizing the fact that qualitative analysis
is "interpretive," the research team met regularly to review and analyze the accumulated data. Validity of the Study Validation procedures included triangulation by multidisciplinary analysis garnered from a variety
