There are a plethora of negative outcomes associated with uncontrolled diabetes and in order to achieve appropriate management of the disease requires that patients should be knowledgeable about self-care and disease management. Standard treatment for type 2 diabetes mellitus consists of both dietary and exercise therapy. The study conducted by Sakamaki, et al was designed to evaluate the effects of a disease management protocol on "various clinical indicators and behavior and quality of life scores of patients with diabetes" (Sakamaki, et al, 2006, p. 46). This program is founded on "physician-initiated and directed disease management" and utilizes a "lifestyle notebook, which sets individualized goals and monitors behavior" (Sakamaki, et al, 2006, p. 46). Hypotheses and research questions The hypothesis in this study is implied by the study design, which suggests that the principal focus of the study was whether or not instructing patients to keep a diary would be instrumental in helping them obtain previously-determined obtainable goals that had been set by their "physicians and allied health professionals based on evaluations of their own lifestyle risks" (Sakamaki, et al, 2006, p. 45). In this case the research questions and the hypotheses are one in the same, as the study was designed to evaluate the efficacy of a specific diabetes management program that was instituted in Japan.
