Airplane accidents are very rare and major crashes are even rarer. In fact, accidents happen so seldom, they are measured in terms of rate per million departures and the data is worldwide. Between 1950 and 2009, there were 1,300 fatal air accidents in the world. This does not include private or military aircraft (PlaneCrashInfo.com, 2010). Of these 29 were due to pilot error, 16 were related to the weather, and 5 were due to mechanical problems that led to pilot error. Weather conditions alone accounted for 12 accidents and mechanical failures accounted for 22 fatal accidents (PlaneCrashInfo.com, 2010). The fatalities per million flight hours is 4.03 on regular airliners (PlaneCrashInfo.com, 2010). More accidents are related to pilot error than to any other single cause. Some pilot errors involved weather conditions or mechanical issues wherein the pilot probably made the wrong decision. This would suggest that pilots need more training. The challenge is that every single possible situation cannot be predicted or even thought about. One thing airlines can do is not over-schedule pilots and other crew members. The rate of accidents has not decreased by any significant amount but air traffic has doubled and tripled over the last thirty years.
