study consists of elderly individuals over the age of 65 with cardiac disease living in the Coney Island area of Brooklyn, New York. Much of the focus of community
nursing is on primary prevention, "defined as both the prevention of disease before it occurs and the reduction of its incidence" (Bethea, 1999). True primary prevention is not possible
with this aggregate, for a qualification for being included in it is history of heart disease. Efforts to prevent other occurrences of the same kinds of events or onset
of related ones (such as stroke) can be seen as qualifying as primary prevention, but may be more suited to being classified as secondary prevention. Ultimately, prevention on several
levels can result in benefits that can be described as being far greater than the sum of their parts. Nursing Diagnoses and Rationale for Including in the Program
The exploratory needs assessment identified at least five nursing diagnoses that the program will directly address: * Proper use of medication; * Importance of medication;
and * Symptoms of stroke or heart attack. A primary goal is that of enhancing prevention, but as stated, the aggregate by definition
already has been diagnosed as having some form of heart disease. In that sense, primary prevention is not possible. The goals of the program focus more on secondary
and tertiary prevention, for which survey data indicates there is a need in the community. Use of Medication In the course of the
