under study consists of elderly individuals with cardiac disease living in the Coney Island area of Brooklyn, New York. The "graying of America" is an established fact, with the
local percentage of people over the age of 65 standing at 12.4 percent (United States, 2003). Both of these figures are expected to grow to be of even greater
effect as the first of the baby boomers approach retirement age. Those adults who now are 65 years old and older are not officially a part of the baby
boomer generation. They are numerous, in part because technological advances have resulted in longer lives for many who could not have survived life-threatening events - such as a heart
attack - only a generation ago. The model on which the assessment tool is based is the Orem model of self-care. Though
members of the aggregate necessarily need to rely on health care professionals to quantitatively monitor their conditions and to prescribe treatment, the success of that treatment is wholly dependent on
individuals compliance with treatment programs prescribed for them. Aggregate Assessment Tool This tool is adapted from an unpublished community health initiative located in
Erie, Pennsylvania (Minnis, 2002). As is the case here, the aggregate for which this tool was developed is that of persons over the age of 65 with heart disease.
The design of the assessment tool is such that it provides information about individual members of the aggregate, while also providing (or reinforcing) information for those completing the form.
