nobody escapes the effects of this new law. The components of the law will be implemented over a period of several years, some were already effective in 2010. The electronic version of this Act is 906 pages long and this is an amended copy (http://www.gpo.gov/fdsys/pkg/BILLS-111hr3590enr/pdf/BILLS-111hr3590enr.pdf). There are so many clauses, nobody could possibly remember them all and it is doubtful anyone really understands this Act fully. The best anyone can hope for is that experts, health care providers, insurance companies, and government officials are conversant with the components that directly impact and mandate their actions. The Act includes requirements for every kind of facility. For example, requirements for a nursing facility states that within three years of implementation, it shall "have in operation a compliance and ethics program that is effective in preventing and detecting criminal, civil, and administrative violations under this Act and in promoting quality of care consistent with regulations developed under paragraph (2)." 1 The Act itself identifies numerous stakeholders. Some of these are hospitals, patients, researchers, policymakers, home health agencies, experts, Medicare beneficiaries, representatives of nursing facilities, residents of nursing facilities, pharmacy industry, prescription drug plans, state, regional, local leadership, employers, health care providers, health insurers. It defines multi-stakeholder groups as a voluntary collaborative of organizations representing a broad group of stakeholders interested in quality.2 There are layers upon layers of management involved in the implementation and monitoring of this Act. Just a few of the federal agencies include the Center for Medicare and Medicaid, the Social Security Administration, the IRS, the Department of Health and Human Services, and numerous new administrative, advisory, and expert panels that are to be appointed.
