Nurse practitioners and other advanced practice nurses have filled significant roles in US health care for years. They have legal rights and the technical expertise to take on much of the burden of providing direct health care services, generally under the supervision of an MD. This advanced practice niche is a hybrid position arising from increasing pressures on the still-evolving American health care system; it is a position respected in the profession and welcomed by patients. For a variety of reasons, it is in the interest both of the US health care system and the patients it serves to seek to use increasing numbers of advanced-practice nurses, not the least of which is economic. A single practitioner can see only so many patients in a days time, and on basis of cost alone, health care facilities ostensibly could gain the services of one MD or four advanced-practice nurses for the same base expenditure (Greene, 2000). The Nurse Practitioner Nurse Practitioners (NPs) practice in all areas of health care involving direct patient care. The role originated in the United States and since has spread to "more than 30 countries from Europe to Australasia" (Walsh, 2003; p. 22). The intended role is that of partner with an MD in providing direct patient care in terms of serving in roles that formerly had been reserved for MDs alone, such as prescribing ethical drugs and providing direct advanced treatment. The NP is to work under the supervision of an MD, but that supervision typically is rather loose. It is administrative supervision, and one in which accountability for gross error includes the MD as well as the NP. ...
