Introduction Access to healthcare for the rural elderly is the result of the combination of a variety of factors, each of which represents a complex issue. As the first of baby boomers already have entered retirement age with the great wave yet to come, "now" is an appropriate time to rectify the issues that exist. Background Many issues have equal effect on rural and urban practitioners and elderly patients. Though there may be a higher number of practitioners in urban areas and transportation may (or may not) be less of an issue, changes in Medicare rules and social programs affect virtually all elderly patients across geographical lines though those in rural areas are most negatively affected. Accessibility to and the cost of health care have been overriding issues in healthcare in recent decades. We have seen the advent and growth of health maintenance organizations (HMOs), the explosive growth of Medicare and Medicare abuses and the resulting "crackdown" on Medicare policies and procedures. Though there has been great debate over how to manage health care in the US, it has been relatively recently that the question has been raised that perhaps access to health care is not a right as has been assumed, but rather a privilege that belongs only to those who can pay for their own care or arrange for a third-party payer to pay the bulk of the cost.
