1. Introduction Academic Medical Hospital (AMH) is a teaching hospital, associated with the local university in a rural area. The role in the community is important, and in line
with the role as a teaching hospital as well as in consideration of the patient needs, it is important that the hospital adopts best practice. The hospital has seen a
number of initiatives over the years, many of which have failed. The latest initiative is that of Six Sigma in the emergency department (ED). The initial results appear to be
positive, with significant reductions in waiting times. The project has gained the support of some staff, but others remain very reluctant and resultant to the introduction of the latest new
measure which is hindered by the failure of the project to gain the support of the projects sponsor. The project is nearing the last stage of implementation and there are
decisions which need to be made concern the way project will move forward, these include the way that the current results are reported, assessing the actual statistical changes, looking at
the current obstacles and considering the way that the project may deal with these issues and move forward in the most effective manner. This is an important time for AMH
as the system can be rolled into other departments. 2. Current Issues and Opportunities There appears to be mixed attitudes concerning the implementation of the Six Sigma project, while
the project has had the support of the senior management and the administrative staff as seen with the initial meeting where it is fair to assume that many of the
