(Nettina, 2005). A principal attraction of advanced practice nursing for many nursing professionals is the fact that APNs have the ability to influence the culture and practice of health care
organizations, bring a higher level of health care to American communities. For instance, Donnelly (2002) describes a small, urban nurse-managed health center in which APNs are in charge of
primary care. To mitigate the reservations of a community resident regarding this, the resident was encouraged to make an appointment for a routine physical. Donnelly records the residents reaction: "I
have never had such a complete examination and explanation of the results...Ill be back" (2002, p. V). Clearly, the APNs at this clinic are influencing health care in a positive
manner; but, it is also true that APN can implement change that facilitates this process. Elser, McClanahan and Green (1996) describe how APNs acted as change agents within a
clinical setting by establishing a research-based minimal stimulation protocol for very low-birthweight (VLBW) infants. This protocol demonstrates how APNs can function as change agents within their workplace. APNs receive as
part of their academic preparation knowledge that pertains to how "to initiate, plan and manage change" (Elser, McClanahan and Green, 1996, p. 72). When change is planned, it results from
conscious effort to make a positive adjustment within a health care system. First of all the APNs in this case recognized the need to solve a problem, i.e., the over-stimulation
of the neonate. They then developed a strategic plan, the protocol, and educated the staff, via a prepared video, implementing these changes (Elser, McClanahan and Green, 1996). The change was
