These days, of course, medicine consists of not only primary care physicians (or "generalists") but also all kinds of specialists that can treat
everything from a wart on the pinkie toe to the ravages of the worst kinds of cancer. Technology, Howell points out, is one reason why medicine has become so advanced
and so splintered. Technology is also one reason why healthcare has become so expensive. The United States has one of the most
technologically advanced healthcare systems in the world. Less than a century ago, a diagnosis of breast cancer was considered a death sentence. These days there are more breast cancer survivors
than there are fatalities. Measles, mumps an chickenpox, once considered fatal in children, can be controlled by vaccines; and even if children catch these diseases, they arent death sentences any
more. Even "old age" diseases like strokes and heart attacks dont phase medicine; many people survive well into their 80s and beyond.
But all of this wonderful and marvelous medical technology comes with a price - and that price is that not everyone has access to the wonderful and marvelous technology. If
an individual is fortunate enough to have employer-based insurance that covers most of a mammogram, ultrasound and chemotherapy necessary to treat breast cancer, that person is fortunate. But there are
others who arent employed, or who dont have employer-based insurance and who are earning to much to qualify for Medicaid. Paying out of pocket in such a situation isnt really
