When other major health considerations (diverticulosis) are ruled out, it is necessary to consider the possibility that mitigating factors, including her use of certain drugs (acetaminophen with codeine and ferrous
sulfate) may be causing constipation that are having a negative impact on her existing hermorrhoids. Other factors, including hypoactive bowel sounds and her self-reporting of a diminishing frequency of
bowel movements, all determine the direction of a treatment plan. In understanding the treatment options and the focus of the treatment process, it is necessary to develop some desirable
clinical outcomes for the patient and create a rationale for the treatment plan for this patient. Clinical Outcomes The clinical outcomes for the patient should relate both
to the patients concerns and to existing health issues. The patient related alarm over the presence of blood in their stool, though they did not complain of extenuating pain.
In addition, the patient reported increasing constipation, presented through a decreasing number of bowel movements each week over the past 5-6 weeks. From the patients perspective, then, two
beneficial outcomes of a treatment process would be an increased number of bowel movements in the course of a week and decreased presence of rectal bleeding. From a
clinical perspective, there are additional considerations. An assessment of the patient determined a presence of mild anemia and migraine headaches, both of which are being treated through the use
of pharmacological interventions. The mild anemia is being treated with ferrous sulfate and the headaches are being treated with acetaminophen with codeine. Unfortunately, these treatments may be increasing
the constipation for this patient, and clinical outcomes need to reflect an awareness of the continued management of these two health issues in addition to addressing the patients concerns.
